This policy applies to all students enrolled in BSN, MSN, DNP, and post-master's certificate programs. For admission and progression in the College of Nursing, students must provide proof that they are free of the communicable diseases in this policy and follow the health requirements of our clinical agencies, as outlined below. A licensed clinician must complete, sign and date the Immunization and Physical Compliance Form, available from the Office of Student Services. Each student is responsible for submitting the following to the College of Nursing via a third party vendor who is approved to track and maintain student health and other legal documents:

  1. Certification in cardiopulmonary resuscitation (CPR): Student must provide a copy of a current CPR card. Certification must be Basic Life Support (BLS) for Healthcare Provider through the American Heart Association ONLY.
  2. RN Licensure: Students enrolled in the RN-BSN, MSN, DNP, or post-master's certificate programs must have an active, unrestricted RN license in either: the State of Ohio for students taking on-campus courses or who will be completing practica in Ohio; or in the state of residence and practice for applicants residing and completing practica outside of the State of Ohio. Proof of the appropriate licensure must be obtained within the first semester of the student's program, or prior to beginning practicum hours, whichever comes first.
  3. Background Check: Students are required to submit Federal and State criminal background checks through the College's vendor.

In accordance with HIPAA rules and regulations, these records will be maintained by the College's vendor.

  1. Mantoux tuberculin (Tb) skin test: Upon admission students must obtain the TB test using the two-step method or QuantiFERON blood test. The two-step method means that student will receive a first Tb skin test and, one week later, will receive the second Tb skin test. Results of EACH step must be submitted.
    1. Results of a one-step Mantoux Tb skin test or QuantiFERON blood test must be submitted each year thereafter while the student remains in the nursing program.
    2. If the student has a positive reaction to Tb, he/she is required to have a chest X-ray, or indicate date of last chest x-ray if it was completed within the past one year. Each year thereafter the student will be required to complete an Annual Mantoux/TB Screening Form, which can be obtained from the Office of Student Services.
  2. Rubeola (Measles), Mumps and Rubella (MMR): A positive titre is required for each disease. A negative titre will require the administration of an MMR booster.
  3. Adult Diphtheria Tetanus: A Tdap is required, regardless of interval since last Adult Tetanus. Adult Tetanus is only accepted with proof of a Tdap. Both vaccines are valid for 10 years.
  4. Hepatitis B vaccination: This vaccination is highly recommended. Dates of all three injections are required. A titre-indicating immunity is also acceptable. If the student chooses not to receive the Hepatitis B vaccination, the waiver section must be signed and dated. Since Hepatitis B is a series of three injections over the course of several months, it may not be completed before the deadline. Students should start the series immediately and submit an updated Immunization and Physical form once future injections are completed.
  5. Varicella: A positive titre is required. A negative titre requires proof of two vaccines administered in the lifetime.
  6. Flu Shot: Annual Influenza vaccine is required regardless of clinical rotation status. A licensed clinician-signed Influenza waiver is acceptable for allergies at some clinical agencies.
  7. Drug Toxicology: Some clinical agencies require 10-panel drug screen. Drug screens are valid for 1 year only. Students will be contacted prior to beginning clinical at an agency that requires a drug screen.
  8. Physical examination is required within one year prior to admission to the program. The physical must indicate that the student can perform the duties of a nurse and is free of all communicable diseases.

Students who have allergic reactions or other physiological conditions that preclude them from receiving vaccinations are required to contact the Office of Student Services upon admission to their academic program. Such conditions may prevent students from completing clinical/practical coursework at some partner agencies.

It is the student's responsibility to maintain all immunizations as needed, a valid CPR certification, background checks, and for graduate and RN-BSN students, a valid RN licensure. Students are required to demonstrate that they are compliant for twelve months based upon the term of admission.

Students who are not compliant with this policy by the first semester of enrollment in their respective major will not be permitted to continue in the program until compliant. If a student becomes noncompliant during a term, they will not be permitted to attend clinical/practicum until the student demonstrates proof of compliance. Any missed clinical due to non-compliance will be considered a clinical failure.
